Who Qualifies for Our Program?
You may be eligible for Prestigious Supportive Housing if you:
✓ Are medically stable (not requiring active hospital-level care)
✓ Need short-term supportive housing to continue recovering
✓ Are committed to working toward independence and permanent housing
✓ Are willing to participate in case management and recovery planning
We serve:

Individuals discharged from hospitals with no stable housing

People experiencing homelessness who are ready to recover

Those recovering from illness, injury, or surgery

Anyone needing a safe bridge between crisis and independence
We welcome referrals from hospitals, social workers, case managers, family members, and self-referrals.

The Application Process
Step 1: Contact Our Intake Team
Call us at 424-333-4183 or email info@prestigioussupportivehousing.com. We're available 24/7 to take your call.
Step 2: Complete a Brief Screening
We'll ask about your current situation, medical status, housing needs, and goals. This takes 10-15 minutes.
Step 3: Intake Assessment
If our program is a good fit, we'll schedule an in-person or virtual assessment to finalize your care plan and housing placement.
Step 4: Move In
Once approved, you can move into your safe, supportive housing and begin your recovery journey immediately.
Typical Timeline: Many clients can be placed within 24-48 hours of completing the intake assessment, depending on bed availability.

Referring a Client?
Hospitals, social workers, case managers, and family members can refer individuals directly to our program.

We respond to all referral inquiries within 2 hours during business hours and within 4 hours after hours.
